IM/IV infusion over 30 mins
【Adult】
●Intra-abdomina infection, neutropenic fever, pneumonia, skin and soft tissue infection, urinary tract infection:
* Mild infection: IM/IV, 1g q12h.
* Severe infection: IV, 2g q12h. or 2g q8h.
○The treatment period is usually 7-10 days, and severe infections may require longer treatment. MAX: 2g q8h.
【Pediatric】
*>40 kg: Refer to adult dosing.
*≧2 months ~≦40 kg: IM/IV, (Mild) 50 mg/kg/dose q12h; (Severe) 50 mg/kg/dose q8h. MAX: 2000 mg/dose.
*1-2 months(使用Cefepime之經驗相當有限): IM/IV, 30 mg/kg/dose q8-12h.

Contraindication
|
Hypersensitivity to cefepime, other cephalosporins, penicillins, other beta-lactam antibiotics, or any component of the formulation.

Precaution
|
Elevated INR; Superinfection; Seizure disorders; Elderly
|

Adverse reaction
|
【>10%】Positive direct Coombs test.
【1% to 10%】Localized phlebitis, pruritus, skin rash, hypophosphatemia, diarrhea, nausea, vomiting, abnormal partial thromboplastin time, change in prothrombin time, eosinophilia, increased serum alanine aminotransferase, increased serum aspartate aminotransferase, hypersensitivity reaction, headache, fever
【<1%】Erythema of skin, urticaria, hypercalcemia, hyperkalemia, hyperphosphatemia, hypocalcemia, Clostridioides difficile colitis, oral candidiasis, vaginitis, anemia, increased alkaline phosphatase, increased serum bilirubin, local inflammation, local pain, increased blood urea nitrogen, increased serum creatinine

Notes
|
1.Cefepime調配後溶液的顏色則呈無色至琥珀色，粉末及溶液在儲存時，可能顏色會稍微變深，然對其效價並無影響。
2.Store intact vials at 20-25°C. Protect from light. After reconstitution, stable in NS and D5W for 24 hours at 20-25°C and 7 days at 2-8°C.

給藥及保存方式 |
用法:IV滴>30min、IM, IV推3-5min
以注射用水或NS配製如下:
IM,加2.4ml→3.6ml
IV滴,加10ml→11.3ml
取需要劑量加入相容點滴中。
IV滴濃度1-40mg/ml。
配製後室溫可放24hr,冷藏7天。
