【Adult and pediatric≧40 kg】
●Usual dose: depends on severity
*Mild-moderate: 250mg q8h or 500mg q12h
*Mild-moderate lower respiratory tract: 500mg q8h or 875mg q12h
*Severe (step-down): 500mg q8h or 875mg q12h
●Ear, nose, GU tract, throat or SSTI:
*Mild-moderate: 500mg q12h or 250mg q8h
*Severe: 875mg q12h or 500mg q8h
●H. pylori eradication: in combination with other agents depends on regimens
*Clarithromycin triple Rx: 1g Q12H (+ clarithromycin + PPI) for 14 days.
*Concomitant Rx: 1g Q12H (+clarithromycin + metronidazole + PPI) for 10-14 days.
*Sequential Rx: 1g q12h (+PPI for 5-7 days; then, follow by clarithromycin + metronidazole + PPI for 5-7 days).
*Hybrid Rx: 1g q12h (+PPI for 7 days ; and follow with amoxicillin 1g + clarithromycin + metronidazole + PPI for 7 days).
●Lower respiratory tract infection:875mg q12h or 500mg q8h
●Pharyngitis, group A streptococci:500mg q12h (max:1g/d) for 10 days
【Pediatric】Infants, Children , Adolescents (<40kg),
●Mild-moderate infection: 25-50mg/kg/day divided in q8h (Max: 500mg/dose)
●Severe infection (step-down): 80-100mg/kg/day divided in q8h (Max: 500mg/dose)
●H.pylori eradication: Children, Adolescents, 50mg/kg/day divided in 2 doses for 10-14 days (Max:2g/d)(+ PPI or bismuth, and + clarithromycin +/-
metronidazole.
●Acute otitis media (age≧2mo):80-90mg/kg/day divided in q12h (max: 4g/d)
●Pneumonia,CAP:(age≧3mo)(max:4g/d)
*Empirical treatment: 90mg/kg/day divided in q12h
*Group A Streptococcus:50-75mg/kg/day divided in q12h
*H. influenzae:75-100mg/kg/day divided in q8h
*S. pneumoniae (PCN MIC≦2 mcg/mL), mild infection or step-down:90mg/kg/day divided in q12h or 45mg/kg/day divided in q8h
●Rhinosinusitis, acute bacterial:
*Low dose:45mg/kg/day divided in q12h
*High dose:80-90mg/kg/day divided in q12h
●Tonsillopharyngitis:50mg/kg QD or 25mg/kg q12h 10 days
●UTI,prophylaxis (VUR):≦2 mo,10-15mg/kg QD

【unknown】Hypersensitivity angiitis, agitation, anxiety, behavioral changes, confusion, dizziness, headache, hyperactivity (reversible), insomnia, seizure, acute generalized exanthematous pustulosis, erythematous maculopapular rash, erythema multiforme, exfoliative dermatitis, SJS, TEN, urticaria, dental discoloration (brown, yellow, or gray; rare), diarrhea, hemorrhagic colitis, melanoglossia, mucocutaneous candidiasis, nausea, pseudomembranous colitis, vomiting, crystalluria, agranulocytosis, anemia, eosinophilia, hemolytic anemia, leukopenia,thrombocytopenia, thrombocytopenia purpura, cholestatic hepatitis, cholestatic jaundice, hepatitis (acute cytolytic), increased serum ALT/AST, anaphylaxis, serum sickness-like reaction.

使用第一線抗生素須注意依體重,肝腎功能調整劑量,依感染部位選用合適抗生素,且下列感染情況不宜使用: (1) 有多種器官嚴重疾病或感染可能之危急病人(2) 第一線狹效抗生素使用3 天無效 (3) 社區感染無正當理由仍使用明顯療效不足之藥
