【Adult】
●Individualize the Janumet dosage on the basis of the patient’s current regimen, effectiveness, and tolerability.Max.sitagliptin 100 mg/day and metformin 2000 mg/day.
* Diabetes mellitus type 2, inadequately controlled on metformin monotherapy: initial, sitagliptin 50 mg plus current dose of metformin BID.
* Diabetes mellitus type 2, inadequately controlled on metformin 850 mg BID: initial, (sitagliptin 50 mg/metformin 1000 mg) BID.
*Diabetes mellitus type 2, inadequately controlled on sitagliptin monotherapy: initial, (sitagliptin 50 mg/metformin 500 mg) BID.
* Diabetes mellitus type 2, switching from sitagliptin co-administered with metformin: initial, same as current doses of sitagliptin and metformin.
* Diabetes mellitus type 2, when administered with a sulfonylurea or with insulin: a lower dose of sulfonylurea or insulin may be required.
【Pediatric】Safety and effectiveness have not been established.

|
Iodinated contrast media,intravascular use in radiologic studies, metabolic acidosis, acute or chronic,including diabetic ketoacidosis.renal disease or dysfunction (ie, serum creatinine of 1.5 mg/dL or greater in males; 1.4mg/dL or greater in female.

Precaution
|
【Sitagliptin】arthralgia; bullous pemphigoid; hypersensitivity reactions; pancreatitis; renal effects; cardiovascular disease; renal impairment.
【Metformin】acute or chronic metabolic acidosis.

Adverse reaction
|
【Sitagliptin】
【1-10%】hypoglycemia; nasopharyngitis.
【Unknown】diarrhea, nausea; increased serum creatinine.

【Metformin】
【>10%】diarrhea; nausea and vomiting; flatulence; infection
【1-10%】chest discomfort; flushing; palpitations; headache, dizziness, taste disorder; diaphoresis, nail disease, skin rash; decreased vitamin B12 serum concentrate; hypoglycemia; nausea, dyspepsia, abdominal distress, abnormal stools, constitipation; heartburn; weakness, myalgia; rhinitis, dyspnea; flu-like symptoms, upper respiratory tract infection; accidental injury

藥品說明：
1.TZD、DPP-4抑制劑、SGLT-2抑制劑、或含該成分之複方，限用於已接受過最大耐受劑量的metformin能無法理想控制血糖之DM type II。DPP-4抑制劑、SGLT-2抑制劑及其複方宜擇一使用。
藥品理由：
01 已接受過最大耐受劑量的metformin仍無法理想控制血糖之第二型糖尿病患者
